Role Description:

Under the direct supervision of the Head of Service, be a resource to answer client questions, schedule care assessments and visits and offer expert advice to inform and educate prospective or current clients on any inquiry they may have. Care Specialists will also be asked to perform the duties of a Care Partner and be available to provide activities of daily living (ADL) services and coordination of other disciplines to meet the needs of our clients in various settings that they call home (i.e. - home, hospital, nursing facilities, and assisted living).

Responsibilities Include But Not Limited To:

  • Provide an exceptional customer service experience for both clients and prospects across all points of contact (phone, email, text and chat)
  • Schedule care assessments and client visits
  • Work closely with Care Partners and Care Advisors to provide clients with a seamless service transition
  • Basic ability to use a tablet or laptop, email, text and other technologies
  • Upon completion of licensure, follow a personalized care plan as directed by Care Advisors and Care Partners to perform the following tasks
  • Assist with bathing, grooming and toileting
  • Assist with basic food preparation and grocery shopping
  • Assist with medication reminders
  • Assist with light housekeeping
  • Assist clients with mobility, including lifting and transferring
  • Safety assurance
  • Transportation

Requirements:

  • Be age 18 or older
  • HS Diploma or passage of high school equivalency exam
  • Possess or commit to receiving within 1 year of employment: Certified Medical Assistant, Registered Medical Assistant, Certified Home Health Aide, State Tested Nursing Aide, Licensed Practical Nurse, Registered Nurse or other healthcare provider license
  • Be physically capable of assisting clients with mobility, including lifting and transferring.
  • Possess a current Basic Life Support (BLS) certification from the American Heart Association (AHA), or obtains within 90 days of hire
  • Pass a comprehensive background check and have 2 professional references
  • Pass drug and nicotine screenings
  • Pass a full occupational health assessment that includes strength testing 
  • Have valid in-state driver's license, a car, and auto insurance in your name
  • Be willing to travel to your clients
  • Must demonstrate good judgment, flexibility, initiative, and have a proven ability to problem solve
