Role Description:

In addition to being responsible for providing assistance with activities of daily living (ADL) services and coordinating other disciplines to meet the needs of our clients in various settings that they call home (i.e. - home, hospital, nursing facilities, and assisted living), the Care Advisor assesses each client’s individual care needs and designs specific care plans while maintaining oversight of a team of Care Partners to facilitate a personalized approach geared towards all health conditions with the goal to prevent complications, improve outcomes and enable our clients to continue to live well at home.

Responsibilities Include But Not Limited To:

  • Assess individual clients needs
  • Create personalized care plans
  • Lead a team of Care Partners with responsibilities for schedules, reviews, and service quality
  • Assist with bathing, grooming and toileting
  • Assist with basic food preparation and grocery shopping
  • Assist with medication reminders
  • Assist with light housekeeping
  • Assist clients with mobility, including lifting and transferring
  • Safety assurance
  • Transportation
  • Basic ability to use a tablet or laptop, email, text and other technologies


  • Be age 18 or older
  • HS Diploma or passage of high school equivalency exam
  • Possess one of the following licenses: Certified Medical Assistant, Registered Medical Assistant, Certified Home Health Aide, State Tested Nursing Aide, Licensed Practical Nurse, Registered Nurse or other licensed healthcare provider
  • Be physically capable of assisting clients with mobility, including lifting and transferring
  • Possess a current Basic Life Support (BLS) certification from the American Heart Association (AHA), or obtains within 90 days of hire
  • Pass a comprehensive background check and have 2 professional references
  • Pass drug and nicotine screenings
  • Pass a full occupational health assessment that includes strength training 
  • Have valid in-state driver's license, a car, and auto insurance in your name
  • Be willing to travel to your clients
  • Must demonstrate good judgment, flexibility, initiative, and have a proven ability to problem solve
